Lifeline Forms
The FCC mandates that all Lifeline Government assistance applicants must submit a signed Lifeline Certification
form.
If you placed your order using the on-line ordering process, the
Lifeline form was created as part of that process.
If you placed your order over the phone or, otherwise need to submit a Lifeline certification form for an order
that you have already placed, you can do so here.
There are two methods available for submitting the form, either one is acceptable.
Method 1: Log into our customer portal and complete the form on-line
or,
Method 2: Download the paper form, print it out, fill it out, then return it to us.

Click your state below to go to the customer portal login. Not all states have an on-line
form available so, if yours is not listed, use Method 2.
Once logged in, look for the "Update my Lifeline Information" link in the "Lifeline" section. Follow that
link to submit a new lifeline certification. |

If this is the first time your are using the customer portal,
you will need your customer account number. When you get to the login screen, click the "register" link at the
bottom. Enter your account number, zip code, and email address into the appropriate fields, then click submit.
Your account password will be emailed to you. Use that password to then log into your account.
Once logged in, look for the "Update my Lifeline Information" link in the "Lifeline" section. Follow that
link to submit a new lifeline certification. |
